Climate change is no longer a distant threat-it’s actively reshaping the health landscape of communities worldwide. But understanding exactly how rising temperatures, shifting weather patterns, and environmental changes translate into health outcomes requires grasping several key concepts. Vulnerability, exposure, and adaptive capacity form the foundation of how public health professionals analyze and respond to climate-related health risks. Getting familiar with these terms is essential for anyone looking to understand or work in climate health adaptation.

What is climate change health vulnerability?

Vulnerability in the climate-health context refers to the propensity or predisposition to be adversely affected by climate variability and change. It’s not simply about being exposed to a hazard-it encompasses the full picture of why some populations suffer more severe health consequences than others when faced with the same climate stressor.

Human health vulnerability to climate can be understood as a function of three interconnected factors: sensitivity (how responsive health systems and populations are to climate changes), exposure (the nature and extent of contact with climate hazards), and adaptive capacity (the ability to cope with and adjust to changing conditions). These three elements work together to determine how severely a community might be affected by climate-related health threats.

For healthcare planners and public health officials, vulnerability assessments serve as critical tools. The World Health Organization has published guidance to help countries assess current and future vulnerability to climate-related health risks, which has been applied in more than 50 countries worldwide. These assessments identify which populations and geographic areas face the greatest risks and where health systems need strengthening.

Understanding exposure pathways

Exposure represents the contact point between people and climate-related stressors. These stressors can be biological (disease vectors like mosquitoes), chemical (air pollutants), physical (extreme heat), or psychosocial (displacement-related stress). What makes exposure particularly complex is that it can occur in various ways and timeframes.

Types of climate exposure

Environmental exposure involves the magnitude, frequency, and duration of contact with climate-related risks. A person might experience a single acute event-like a flood or heatwave-or face repeated chronic exposures over time. Exposure can happen in one location or across multiple geographic areas as people move or as climate conditions shift.

The pathways through which climate affects health are numerous. According to WHO data, climate stressors heighten waterborne and foodborne disease risks, with 2 billion people lacking safe drinking water and 600 million suffering from foodborne illnesses annually. Temperature and precipitation changes enhance the spread of vector-borne diseases, while extreme weather events directly cause deaths and injuries.

Direct versus indirect exposure

Direct health impacts occur when climate hazards immediately affect human physiology-heat stress during a heatwave or injuries during a storm. Indirect impacts follow more complex pathways: changing rainfall patterns alter mosquito breeding grounds, which shifts disease transmission; drought reduces crop yields, leading to malnutrition; rising sea levels contaminate freshwater supplies.

Climate change also disrupts the social determinants of health-livelihoods, food security, housing stability, and access to healthcare. These indirect effects often prove more challenging to measure but can be equally devastating for population health.

Sensitivity: why some communities are hit harder

Sensitivity describes how much a population’s health is affected when exposed to a climate hazard. It encompasses physiological and socioeconomic factors that determine the degree to which communities experience health impacts from climate-related changes.

Biological sensitivity factors

Age plays a significant role in sensitivity. Older adults are particularly vulnerable to heat-related illness because their bodies regulate temperature less efficiently. Children face elevated risks from many climate hazards due to developing immune systems and greater physiological sensitivity to environmental stressors. Heat-related deaths among those over 65 have risen by 70% over two decades, illustrating how age-related sensitivity compounds with increasing climate hazards.

Pre-existing health conditions dramatically increase sensitivity. People with cardiovascular disease, respiratory conditions, diabetes, or compromised immune systems face heightened risks during extreme heat events, poor air quality days, and disease outbreaks. Pregnant women represent another sensitive population, with heat exposure linked to adverse birth outcomes.

Social and economic sensitivity

Socioeconomic status profoundly influences sensitivity. Low-income communities often lack resources for protective measures-air conditioning during heat waves, flood insurance, or the financial cushion to absorb climate-related economic shocks. In vulnerable regions, the death rate from extreme weather events in the last decade was 15 times higher than in less vulnerable areas.

Housing quality, occupational exposure, access to healthcare, and social support networks all contribute to sensitivity. Outdoor workers face greater heat exposure; those in substandard housing experience worse outcomes during floods and storms; communities with limited healthcare access struggle to manage climate-related illness surges.

Adaptive capacity: the ability to respond and cope

Adaptive capacity refers to the broad range of responses and adjustments that individuals, communities, and systems can make to climate impacts. It encompasses both the ability to moderate potential damages and the capacity to take advantage of opportunities while coping with consequences.

Individual and community-level adaptation

At the individual level, adaptive capacity includes awareness of risks, financial resources for protective measures, and physical ability to respond to warnings. A person who can afford air conditioning, understands heat risk warnings, and has transportation to cooling centers has greater adaptive capacity than someone without these resources.

Community adaptive capacity depends on infrastructure quality, emergency response systems, social cohesion, and institutional preparedness. Adaptation includes the strategies, policies, and measures undertaken now and in the future to reduce the burden of climate-sensitive health outcomes. An early warning system for heat events exemplifies public health adaptation.

Health system adaptive capacity

Health systems themselves must adapt to climate change. This means building climate-resilient facilities, training healthcare workers to recognize and treat climate-sensitive conditions, developing surge capacity for climate-related emergencies, and integrating climate considerations into public health planning.

A review of vulnerability assessments found that many do not investigate in sufficient detail the climate-sensitivity of broader health system components-facilities, workforce, technologies, and partnerships. This gap can result in insufficient attention to critical vulnerabilities and inadequate attention to measuring health sector impacts.

Documented health impacts of climate change

The evidence connecting climate change to adverse health outcomes continues to mount. Between 2030 and 2050, climate change is expected to cause approximately 250,000 additional deaths per year from undernutrition, malaria, diarrhoea, and heat stress alone. The direct damage costs to health are estimated at $2-4 billion annually by 2030.

Vector-borne disease shifts

Climate impacts the spread of vector-borne diseases by affecting vector populations and disease distribution. Warmer temperatures, altered precipitation patterns, and changing seasons influence how and where disease-carrying mosquitoes and ticks survive and reproduce.

From 1898 to 2016, Anopheles mosquitoes-the vectors for malaria-expanded their range by an average of 6.5 meters elevation per year and moved poleward 4.7 kilometers annually. Longer summers are expected to extend mosquito season by 1 to 2 months by 2050 for disease-causing species responsible for dengue, Zika, yellow fever, and chikungunya.

The 2023 World Malaria Report highlighted climate change as a substantial threat to malaria control. Changes in temperature, humidity, and rainfall influence the behaviour and survival of malaria-carrying mosquitoes, while extreme weather events like heatwaves and flooding directly impact transmission and disease burden.

Extreme heat represents one of the most direct climate-health connections. Recent research attributes 37% of heat-related deaths to human-induced climate change. As heatwaves become more frequent, intense, and prolonged, vulnerable populations face escalating risks.

Food and water security

Climate change affects food availability, quality, and diversity, exacerbating nutrition crises. In 2020, 770 million people faced hunger, predominantly in Africa and Asia. Meanwhile, in 2020, 98 million more people experienced food insecurity compared to the 1981-2010 average.

The equity dimension

Research shows that 3.6 billion people already live in areas highly susceptible to climate change. Despite contributing minimally to global emissions, low-income countries and small island developing states endure the harshest health impacts.

Climate-sensitive health risks disproportionately affect the most vulnerable and disadvantaged groups: women, children, ethnic minorities, poor communities, migrants, displaced persons, older populations, and those with underlying health conditions. This inequity underscores that addressing climate change’s health burden requires prioritizing those who bear the greatest risks while having the fewest resources to protect themselves.

Moving from assessment to action

Understanding vulnerability, exposure, and adaptive capacity isn’t merely academic-it provides the framework for effective public health responses to climate change. Vulnerability assessments identify weaknesses in systems that should protect population health and reveal adaptation options.

The process involves describing current climate-health associations, identifying existing protective measures and their effectiveness, reviewing climate impacts across sectors, estimating future health impacts under different scenarios, and identifying additional adaptation strategies. This systematic approach helps health authorities make evidence-based decisions to increase resilience to current and future climate conditions.

Effective adaptation requires action at multiple levels: strengthening surveillance systems to detect emerging climate-health threats, building early warning systems for extreme weather events, improving healthcare infrastructure resilience, addressing underlying social vulnerabilities, and integrating climate considerations into all aspects of health planning and delivery.
